An Oscar-winning action icon is putting her action chops on the line — and what she takes on should sort the hype from the graft.

Every once in a blue moon, an action thriller surfaces that makes the rest of the genre look like it isn't trying hard enough. The Surgeon is the latest to throw that particular gauntlet down — and shockingly, it's sitting on a pristine, untouched 100% Rotten Tomatoes score after its festival debut. Not exactly what you'd expect from a film that, on paper, sounds like a mad scientist's experiment: take the brutal efficiency of John Wick, graft it to the gory medical intrigue of HBO's The Pitt, and watch what unfolds.

The whole thing is held together — quite literally, considering the plot — by Michelle Yeoh. She's no stranger to action, having battered her way through everything from Supercop and Crouching Tiger, Hidden Dragon to winning an Oscar for Everything Everywhere All at Once. Here, Yeoh is an elite surgeon with 35 years' experience, who finds herself taken hostage and then puts her complex skillset to use not just saving lives, but turning the tables on her captors in some rather inventive, apparently very bloody ways.

The Surgeon made its world premiere at the Toronto International Film Festival before getting snapped up by Sony, and it now has an official cinema release date: 2 April 2027 (mark your diary now, if you're that sort).

Director, Cast, and Studio

This is the first big studio thriller cooked up by Roshan Sethi, who’s not just a director and screenwriter but also a practicing M.D. (because why not, apparently). Sethi’s known for co-creating Fox’s hospital drama The Resident and recently directed a romcom called A Nice Indian Boy, so the mash-up of scalpels and standoffs probably isn’t as random as it sounds.

  • Michelle Yeoh leads the charge as the kidnapped surgeon-turned-action heroine.
  • Martin Freeman (Sherlock, Black Panther) adds some reliable British charm somewhere in the mix.
  • Laurie Davidson (The Sandman) and Yuri Kolokolnikov (Game of Thrones) are in supporting roles.
  • Joseph Mydell (Conclave) rounds out a cast that's just left of typical for this sort of film.

The production is handled by Thunder Road (the folks behind the John Wick franchise, which probably explains the bone-crunching action style) and Tango, with Columbia Pictures handling the wide release.

Genre-Bending, Bloody Mayhem

Critics have apparently been falling over themselves at festivals to heap praise on the thing. According to early reviews, The Surgeon doesn't just stick to one lane: it's equal parts medical drama (though much gorier than usual), (very) tense crime thriller, over-the-top action flick, and — in at least one person's opinion — the sort of bonkers late-night movie that veers close to self-aware parody. Yeoh’s performance in particular is getting singled out, with one reviewer reveling in her ability to dispatch "dozens of baddies" as she anatomically dismantles her way through the runtime.

If you want a slightly warped premise, try imagining what would happen if The Pitt’s Dr. Robby was forced to kill her way out of a hostage situation — apparently that’s about the vibe here, but with more martial arts and arterial spray. Thunder Road may have started this trend with Wick, but The Surgeon seems to be running the concept through a hospital incinerator and coming out the other side with a stethoscope and a body count.

The film’s 100% on Rotten Tomatoes comes after its debut at Toronto — not too shabby for a self-confessed genre mashup that barely anyone saw coming.
